2025 Summary of ACP Recommendations for Preventing Episodic Migraines in Nonpregnant Adults

1. First-Line Treatment: Clinicians should initiate monotherapy with one of the following:

▪️Beta-blockers (metoprolol or propranolol)
▪️Antiseizure medication (valproate)
▪️SNRI (venlafaxine)
▪️Tricyclic antidepressant (amitriptyline)

2. Second-Line Treatment: If the first-line options are not tolerated or ineffective, clinicians should consider monotherapy with:

▪️A CGRP antagonist-gepant (atogepant or rimegepant)
▪️A CGRP monoclonal antibody (eptinezumab, erenumab, fremanezumab, or galcanezumab)

3. Third-Line Treatment: If both first- and second-line treatments fail, clinicians may use monotherapy with the antiseizure medication topiramate.

Topic: Blood test may identify stroke type pre-hospital, enabling rapid intervention

A study to be presented at the annual American Stroke Association International Stroke Conference shows that glial fibrillary acidic protein (GFAP) levels are elevated in patients with intracerebral hemorrhage (ICH) compared with those with ischemic stroke (IS) and stroke mimics. GFAP could thus be an effective prehospital test for assessing brain injury.

The U.S. Food and Drug Administration approved Journavx (suzetrigine) 50 milligram oral tablets, a first-in-class non-opioid analgesic, to treat moderate to severe acute pain in adults. Journavx reduces pain by targeting a pain-signaling pathway involving sodium channels in the peripheral nervous system, before pain signals reach the brain.  
Journavx is the first drug to be approved in this new class of pain management medicines.

Message of condolences

It is with profound sadness that we learned of the sudden tragic passing of Dr. Andualem Dagne, a General and HPB Surgeon at Bahir Dar University’s Tibebe Ghion Specialized Hospital.

Dr. Andualem was a dedicated and talented young surgeon, known for his examplary discipline, extraordinary commitment to his patients and his passion for the field of General and hepatobiliary surgery.

His untimely passing is a profound loss to his family, friends, colleagues, and the entire medical community. He will be missed deeply as he was dearly loved and respected specially by his residents and members of the department.

Our heartfelt condolences go out to his families, friends and all who knew and loved him.

May his soul rest in eternal peace!

Topic: Shorter oral regimens show promise for rifampin-resistant tuberculosis

A phase 3 multinational trial evaluated five 9-month oral regimens for fluoroquinolone-susceptible, rifampin-resistant TB against standard therapy. Four regimens—BCLLfxZ, BLMZ, BDLLfxZ, and DCMZ—were noninferior, though DCMZ did not meet criteria in the per-protocol analysis. Adverse events were comparable across groups, with hepatotoxicity occurring in 11.7%. These findings support shorter, all-oral regimens as effective alternatives, potentially improving adherence and reducing treatment burden.

Topic: FDA clears first in-class test for rapid sepsis diagnosis

Inflammatix received FDA clearance for its TriVerity Test System, a first-in-class molecular test designed to rapidly diagnose bacterial and viral infections and assess illness severity in patients with suspected acute infections or sepsis. FDA's decision was based on the SEPSIS-SHIELD study, which demonstrated high diagnostic and prognostic accuracy in 1,222 patients across 22 sites, regardless of immune status or race.
