Glucocorticoids induced adrenal insufficiency

✅ Essential points

Glucocorticoid exposure via oral administration that poses risk for adrenal insufficiency is expected to at least exceed both of the following thresholds:

👉 Duration of glucocorticoid therapy to pose risk for adrenal insufficiency: 3–4 weeks or greater.

👉 Dose of glucocorticoid therapy to pose risk for adrenal insufficiency: any dose greater than daily hydrocortisone equivalent of 15–25 mg (4–6 mg prednisone or prednisolone, 3–5 mg methylprednisone, 0.25–0.5 mg dexamethasone).

🔴 First Aid for Adults Experiencing Chest Pain: Recommendations

Activate Emergency Response:

▪️Call to summon the emergency medical system (EMS).
▪️Ensure transport to the closest emergency department (ED).

Aspirin Administration

▪️If the individual is alert and experiencing nontraumatic chest pain, encourage chewing and swallowing 162–325 mg aspirin, unless they have a known aspirin allergy.

When to Avoid Aspirin Use:

▪️If there is uncertainty about its appropriateness (e.g., potential contraindications), wait for EMS arrival without administering aspirin.

Epidemiology of diabetes mellitus

Prevalence:

▪️The number of people living with diabetes increased from 200 million in 1990 to 830 million in 2022.
▪️14% of adults aged 18 and older had diabetes, up from 7% in 1990.
▪️Prevalence has risen faster in low- and middle-income countries compared to high-income countries.

Treatment and Medication Coverage:

▪️In 2022, more than half (59% of adults aged 30+) of people living with diabetes did not take medication for their condition.
▪️Treatment coverage was lowest in low- and middle-income countries.

Health Impacts:

In 2021, diabetes caused:
▪️1.6 million direct deaths.
▪️530,000 kidney disease deaths.
▪️11% of cardiovascular deaths.
▪️Nearly half (47%) of diabetes-related deaths occurred before age 70.
