The Role of Acceptance & Commitment Therapy in Medicine

One of my ostomate (a person with stoma) patients whose wound I was treating came close to me and said
' Doctor, I was feeling so lucky and I was grateful that I was able to bit cancer but now I'm saying to myself I should have died as I'm finding it hard to handle my stoma. I'm suffering'

My normal inclination would be to encourage the patient and bring her back to her grateful state, to  tell her how she will get used to everything but I chose not to. I silently sat in her presence as she fought her tears.

Acceptance and commitment therapy helps one to face their emotions as they are and accept the situation as it is with wide open arms. It treasures psychological flexibility. It bases itself in the principle that pain is inevitable. We all have things we wish haven't happened to us but they have happened anyway. It says suffering springs from non-acceptance. If we develop psychological flexibility, we can have pain but we necessarily don't have to suffer.

The thing I chose to do with my patient was to let her pain out, to let her be ungrateful, to speak up her wish to die and end her suffering.

Suppressing that will only make her suffer. Within a few minutes, she was telling me how helpful and loving her children are at her time of need. That was the other side of life - love and a desire to live. My patient shows me how I see life for myself - light and dark. It is not all light or all dark. It's a blend of both. And learning to welcome both is the greatest lesson of a life time. Suffering is when we are addicted to the light and run away from dark. That's when we suffer.

Medicos Research Data Collection Agency

Successful Completion of Data Collection for Cancer-Associated Thrombosis Study

Dear Partners and Followers,

We are thrilled to announce a significant milestone achieved by our team at Medicos Research Data Collection Agency. We have successfully completed the data collection phase for a groundbreaking research study titled:

"Assessing the Comparative Efficacy and Safety of Warfarin and Rivaroxaban for Cancer-Associated Thrombosis: Experience From a Resource-Limited Setting."

This study, conducted on 201 patients who were on follow-up from September 2021 to August 2023, aimed to evaluate the comparative efficacy and safety of Warfarin and Rivaroxaban in managing cancer-associated thrombosis (CAT) in resource-limited settings. The findings revealed that Rivaroxaban was more efficacious and safer than Warfarin for patients with CAT, aligning with reports from resource-abundant countries and recommendations from major international societies.

This research provides critical insights into optimizing treatment strategies for CAT, particularly in settings with limited resources, and has the potential to significantly improve patient outcomes.

For full details of the research findings, please visit: https://lnkd.in/dqxyFp7s

We would like to extend our heartfelt gratitude to Dr. Abel Tenaw and his colleagues for their exceptional collaboration and trust in our team throughout this project.

Thank You Hakim podcast for interviewing Dr. Thut Teshome, one of the few general and pediatric anesthesiologist in East Africa.

I feel privileged to have had the opportunity to learn from you though it was only one month at Cure hospital.

Your hardwork, commitment, communication skill was exceptional and infact your smile was magnificence. I love the way you treat pain, which is one of the neglected part of medical care in our practice.

I really injoyed and you will not regret to watch her life journy.

A Call for Health Sector Reform

The health sector receives significant financial investment and plays a vital role in society. However, despite substantial funding, it continues to face major challenges in resource utilization and workforce management. There are persistent inefficiencies in fund distribution at various levels, leading to mismanagement and wastage that deprive essential health services of necessary support.

This issue is particularly concerning as key international donors are reportedly reducing their aid. As a result, the sector must prioritize self-reliance and strengthen local capacities to manage emergencies and outbreaks effectively. The focus should now shift toward investing in local specialists, securing essential medicines, and ensuring the availability of emergency medical equipment. Relying solely on external assistance is no longer a sustainable option.

Furthermore, the limited resources available must be allocated wisely. Budget priorities should be guided by expert recommendations rather than unnecessary expenditures. In the past, hospitals that won performance-based rewards reportedly spent funds on luxury items instead of improving patient care. Such misallocation of resources must be addressed to ensure that every investment directly benefits the healthcare system.

To enhance accountability, a robust monitoring and control system should be established to track revenue collection and fund allocation. Financial losses often occur under the pretense of renovation projects, diverting resources away from patient care. Strengthening financial oversight mechanisms will help prevent such mismanagement and ensure that funds are used efficiently.

The decline of the health sector is not due to a lack of resources but rather systemic challenges in management, innovation, and accountability. Solutions must be driven by professionals committed to improving service delivery. It is essential to remove bureaucratic barriers that delay decision-making and hinder the efficient functioning of healthcare facilities. People in rural and underserved areas continue to face significant challenges in accessing essential care, highlighting the urgent need for reform.

The way forward requires decisive action to eliminate inefficiencies and rebuild a healthcare system that serves the public effectively. Now is the time to prioritize responsibility, transparency, and strategic leadership to create a stronger and more resilient health sector.
